The right help at the right time!SERVICE LOCATION VS BILLING LOCATION IN CLAIM FORMSThe bottom of each HCFA claim form (Boxes #32 and #33) contains address information. However, there is a difference between the two boxes. The information in Box #32 is identified as ‘Service Facility Location Information’, or the location where services were rendered. Providers of service (namely physicians) must identify the supplier’s name, address and zip code. Also, the name and addresses of facility where services were rendered identifies the site where service(s) were provided.
Information in Box #33 identified as ‘Billing Provider Information Location’, or the provider that is requesting to be paid for the services rendered. The information identifies the Provider that is requesting to be paid for the services rendered and should always be completed. In short, the Provider name and address information in Box #33 is indicating where payment should be sent. If address information on claim forms is in conflict with information in CIGNA's processing systems, claim may be returned to Providers. To ensure timely processing of claims and receipt of correspondence, please verify correct address information on claim forms.
