The right help at the right time!

HOME OFFICE ATTESTATION FORM

In an effort to meet the access needs of CIGNA's customers and maintain a network of practitioners with diverse specialties, CIGNA will contract with practitioners working out of a home office. You can find CIGNA's criteria for a home office in the Medical Management Program on our website, www.cignabehavioral.com. The information is located in the ‘Practitioner Network’ section on page 6. Those requirements include:

• The office must be used solely for practice.
• Any patient areas (treatment rooms, waiting rooms, bathroom etc) must be clean and free of smoke, animals (except service) and personal effects.
• The office must have a waiting room separate from the living area that offers privacy from the treatment room during appointment hours.
• The office telephone line must be separate line from the residential line.
• Medical records must be secured in a locked file and/ or room, out of site of patients.
• The office must have parking available for patients.

CIGNA must be made aware if a practitioner if working out of a home office as a Home Office Attestation Form is required. The Attestation Form is a brief questionnaire about the home office. Once the Attestation Form is completed a note will be made that the office meets requirements and the Form will be kept in your file. If you currently work out of a home office and have not completed the Home Office Attestation Form, or if you plan to open a home office please contact CIGNA to request that one be sent to you. We look forward to our continued partnership.
