COORDINATION OF CARECoordination of care between a patient’s Primary Care Physician (PCP) and behavioral health practitioner(s) is crucial to effective treatment. CIGNA provides tools to assist in facilitating communication among the PCP and other behavioral health practitioners. The Medical Management Program (MMP), which can be located on our website, www.cignabehavioral.com, includes an appendix dedicated to coordination of care. Appendix G of the MMP includes a sample release of information, introductory letter and communication form. Additionally, a coordination of care form, which may be used to document care coordination efforts in your treatment records, can be found under Clinical Resources on our website. If you are selected for a high volume medical record review, you may be asked to confirm that appropriate coordination of care was attempted with the PCP and/ or other behavioral health practitioner, if any, during treatment. For more information about CIGNA's policies on treatment record keeping, please refer to the MMP, Section 5, pages 30- 31. Additionally, Clinical Screening Tools are available as a resource. The tools and instructions for use are available for the Patient Health Questionnaire - 9 and the Mood Disorders Questionnaire, in the Clinical Resources section of the website.
