Welcome to Cigna's Provider Survey page. This survey lets you and the members of your household communicate with us about our providers.
There are two short segments of questions as well as space for free form comments.
Office and Appointment Availability Feedback assesses information about the provider outside of the actual treatment. This includes the office and facility, wait times and timeliness of appointments. This part of the survey can be taken at anytime during the course of treatment but is limited to one submission per day.
Treatment Feedback assesses your satisfaction with the treatment provided by the provider. This part of the survey should be taken after completion of treatment and is limited to one submission every six months.
